Help in php with mysql

Dear Sirs, good evening. I need some help with my first system. The scenario is as follows, I need to create a form, where I have a text to search and select the name of a person, and then I need to open some fields where you will be informed of two or more information and write to the mysql database, to bring this information from the bank also if someone can guide me accepted suggestions.
