How can I store questions in this style:

In the bank with these spaces to fill and be able to use these spaces in android for example.

You could use some specific markup to indicate the use of the "__" set and write to the base with this markup. When the app recovers you could replace it with the real characters again.
Example:
#### is alone in the park, sitting by ####. (She/Her/Herself)
