It is always the DHCP server. The point is that as the internet is a network, there are several layers that can change this. For example. if your pc is connected directly to the modem, your ISP's DHCP will give you your IP, if there is a router in the middle (and this is also true for those who are also modems) then the router has a small DHCP server and it will create another internal network and give you an IP of that network (and for the internet, all the PC's of that subnet have the same IP).
Providers usually provide dynamic IP addresses for residential customers, meaning your IP changes over time on the internet, but you can usually request static IP as an extra service from your provider, and of course, for a price.
